Vendor Applications 

Please fill out registration below:

**Booths are available at NO CHARGE.  We ask all exhibitors to bring something to donate
for a door prize.  Food vendors are responsible for paying the $35 for the Health Department
permit.  All vendors keep the profit of the sales.  TCSDHH will coordinator the health permits. 

Applications must be submitted by October 15th, 2008.  We will contact you by October 31st
to confirm your application.
